Work directly with the medical staff to provide medication therapy assessment, consultation and management
Manage medication therapies via medical staff approved collaborative practice agreements. These include: anemia management protocol, anticoagulation therapies, antimicrobial stewardship program, pharmacokinetic analysis, parenteral nutrition, etc.
Clinical pharmacy activities are performed by staff throughout the hospital in our decentralized patient care area locations
Daily team rounding
Specialty pharmacy services include: Emergency Room Pharmacist, Oncology Pharmacist, Pediatrics Pharmacist, Critical Pharmacist, Operating Room Pharmacist, Telemetry Unit Pharmacist
Participation in clinical quality committees, medication safety committee and medical staff committees
10-hour shifts available, rotating weekends

EDUCATION: Graduate of a college of pharmacy accredited by the American Council on Pharmaceutical Education (ACPE).
REQUIREMENTS: Current California Pharmacist License in good standing with the California Board of Pharmacy. Two years of hospital experience preferred. Hospital pharmacy residency preferred. Must possess a specialized pharmaceutical knowledge in addition to a good working knowledge of pharmacokinetic dosing and nutritional support. Effective verbal and written communication skills. Computer working knowledge.
