Job Description
Enters all required patient demographic information.
Enters correct charge description
Enters correct medication: Description ; Strength; Route; Expiration Date; Start/Stop Dates
Enters mg/kg dosing for peds/neonates.
Verifies all technician computer entries.
Corrects all orders that are entered in error or may be misunderstood.
Performs computer backup.
Posts debits/credits accurately.
Enters metric dosage.
Maintains order entry error rate < 2%.
Serious order entry error rate < 5%.
Capable of entering a minimum of 100 orders for 10 hour shift.
Averages at least 70 orders entered per 10-hour order entry shift.
Generates required reports: MAR, Pick IV, production reports, ADI, etc.
Alerts system analyst of any problems with system software or hardware.
Able to look up a patient's location in computer system.
Able to look up a patient's demographic information such as height, weight, allergy, Lab information, etc. in computer system.
Able to run patient census and location lists from computer.
Able to refill, load and unload medications.
Able to run reports.
Able to locate and resolve discrepancies.
Able to obtain drug information from Micromedics.
Automix: Enters correct information in Automix.; Performs QC checks.; Inservices other staff members.
Can use the Pharmacokinetics computer program to calculate appropriate medication doses: Adult; Pediatric; Neonate; Geriatric ; Adolescent
Uses the chemotherapy computer system program to accurately calculate chemotherapy doses: Adult; Pediatric; Neonate; Geriatric ; Adolescent
Assists technicians in performing technical tasks.
Accurately completes IV rounds.
Maintains aseptic technique.
Recycles product as appropriate.
Labels containers accurately including auxiliary labels.
Prioritizes work (i.e. Stats made first).
Assists techs in restocking shelves and rotating stock.
Ensures tech maintains IV refrigerator temp. and hood cleaning logs.
Checks technician prepared products prior to release.
Enters correct patient date: Adult ; Pediatric; Neonatel; Geriatric; Adolescent
Assist technician in TPN prep as needed.
Logs correct solutions
Performs all required safety checks.
Completes all documentation.
Takes required protective measures.
Ensures technicians take all required protective measures.
Disposes of chemotherapy waste properly.
Fills orders with correct medication.
Check support staff accuracy prior to medication release.
Prioritizes work to prepare stat, now and waiting in order of importance.
Follows through on fill on arrival orders.
Fills requisitions from nursing floors in a timely fashion.
Prepares outpatients prescriptions: Processes prescriptions within five minutes of start.; Fills out Pharmacy Profile for employees and IMS patients.; Prescription label and blank contains all legal requirements; Counsels patients on Rx medication use as required by law.
Checks for computer entry errors and corrects.
Checks cassettes for accuracy if filled by a technician.
Less than 3 filling errors per 75 cassettes filled.
Completes interaction log.
Makes sure technician completes daily logs: refrigerator, hood cleaning, chemotherapy, etc.
Maintains batch logs.
Maintains appropriate controlled substance records.
Follows non-formulary drug procedures.
Completes notification reports regarding med errors or other incidents.
Signs memos.
Maintains Education log.
Initials charge tickets and places them in correct location.
Documents as required.
Orders medications when stock is low.
Keeps purchasing agent informed of abnormal use of items.
Loans and borrows medications per policy.
Can safely use departmental equipment.
Disposes of waste in the proper containers.
Able to locate: MDS sheets, disaster manual, safety manual, Infection control, P&P, etc.
Demonstrates fire procedure verbally.
Performs floor stock inspections accurately.
Performs outdate checks.
Assists in training of new staff.
Helps other areas when needed.
Able to refill and bill emergency Med Carts.
Updates crash cart stickers.
Maintains security of pharmacy.
Maintains security of controlled substances.
Supervises activities of support staff effectively.
Provides pharmacy management with feedback regarding activities of support staff.
Works with others pharmacists and technicians to settle work priority disputes.
Informs oncoming staff of pending problems, physician call back, etc.
Reviews patient profile to ensure appropriate and safe drug therapy: No therapeutic duplication.; Prevents significant drug interactions.; Prevents avoidable ADR secondary to allergies.
Medication ordered is appropriate for patient ht, wt, age, renal function and disease state
Responds to codes and is an active member of code team.
Pharmacokinetic Dosing Service: Apriori dosing is appropriatel; Interprets levels appropriately.; Modifies does appropriately.; Writes progress notes per protocol.
TPN screening: Contacts physician on any unusual order.; Familiar with normal concentration of adds.; Capable of calculating fluid, electrolyte and calorie needs.; Monitors patient for appropriateness of formulation.
Clarifies controversial orders with physician.
Reviews patient chart for requisite background information prior to contacting physician.
Drug Information: Keeps current on new drugs.; Answers questions timely and accurately.; Provides continuing education lecture, written report, public Ed lecture, Rx forum article or Hospital sponsored event at least four times a year.
Informs oncoming shift of patients requiring Rx monitoring or follow up.
Continuously assesses and improves the performance of care and services provided.
Demonstrated knowledge of CQI through application of the principles in the department.
Uses systematic methods to improve services, plan changes and evaluate outcomes.
Demonstrates effective problem solving techniques and promotes same in others.
Actively supports the mission, goals and objectives of the hospital and department through CQI activities.
Demonstrates expertise in area of specialty.
Serves as a departmental resource for other staff members in the area of specialization.
Actively works to improve knowledge of the rest of the staff in the area of specialization.
Attends and contributes to assigned hospital committees.
Completes special projects on time.

Minimum Education: Graduate of a college of pharmacy accredited by the American Council on pharmaceutical education (ACPE). Doctor of pharmacy degree preferred.
Minimum Experience: Two years of hospital experience preferred. Hospital pharmacy residency preferred.
Preferred Skills and Abilities: Must possess a specialized pharmaceutical knowledge in addition to a good working knowledge of pharmacokinetic dosing and nutritional support. Effective verbal and written communication skills. Computer working knowledge.

Possesses the skills and knowledge to provide all aspects of pharmaceutical care for all PEDIATRIC, ADOLESCENT, GERIATRIC, ADULT patients, including calculating medication dosages, monitoring the effects of medication and outcomes of therapy; The Essential Functions of this job include, but may not be limited to those listed in this job description.
