The Rehab Admissions Liaison is responsible for obtaining referral information, assessing potential patients, discussing Rehabilitation Program with family members, verifying of insurance/Medicare coverage and to coordinate all aspects of the admission process. He/she establishes and maintains regular contact with referral sources, appropriate agencies, and professional associates.


EDUCATION / TRAINING / EXPERIENCE:



Minimum:



· Licensed RN, PT, OT, or SLP in the State of Georgia

· Demonstrates effective verbal and written communication skills

· Demonstrates tactful and effective interpersonal skills



Desired:



· Minimum two years' experience in clinical or Rehabilitation setting

· 1 years experience in interacting with insurance case managers and obtaining pre-certifications



LICENSES / CERTIFICATIONS:



GA Licensed in applicable clinical discipline



SKILLS / KNOWLEDGE / ABILITIES:



Strong working knowledge of Rehab field
