Qualifications: Current CA RN license and AHA BLS Certification. One (1) year recent (within past three (3) years) full-time equivalent experience in a licensed Home Health and/or Hospice agency as an RN/PHN. Current California Driver's License and a clear driving record for the past two years. Proof of automobile insurance as required by law. Consistently supports compliance and the Principles of Responsibility (Kaiser Permanente's Code of Conduct) by maintaining the privacy and confidentiality of information, protecting the assets of the organization, acting with ethics and integrity, reporting non-compliance, and adhering to applicable federal, state and local laws and regulations, accreditation and licenser requirements (if applicable), and Kaiser Permanente's policies and procedures. Consistently demonstrating the knowledge, skills, abilities, and behaviors necessary to provide superior and culturally sensitive service to each other, to our members, and to purchasers, contracted providers and vendors. Preferred Qualifications: BSN preferred, preferred Hospice/Palliative care experience. Knowledge of Hospice Medicare and jt commission standards.
Notes: Provides clinical visits 4pm-10pm and stand by from 10pm-8:30am, alternates between 3 days/week and 4 days/week, will work rotating weekends.
