Qualifications: Current CA RN license and AHA BLS Certification. One (1) year recent (within past three 3 years) full time equivalent experience in a licensed Home Health and/or Hospice agency as an RN/PHN. Current California Driver's license and a clear driving record for the past two (2) years. Proof of automobile insurance as required by law. Consistently supports compliance and the Principles of Responsibility (Kaiser Permanente's Code of Conduct) by maintaining the privacy and confidentiality of information, protecting the assets of the organization, acting with ethics and integrity, reporting non-compliance, and adhering to applicable federal, state and local laws and regulations, accreditation and licenser requirements (if applicable), and Kaiser Permanente's policies and procedures. Consistently demonstrates the knowledge, skills, abilities, and behaviors necessary to provide superior and culturally sensitive service to each other, to our members, and to customers, contracted providers, and vendors.

Preferred Qualifications: BSN. Two (2) years Med/Surg experience in an acute inpatient facility. Knowledge of Medicare regulations, JCAHO, Title 22 requirements, Home Health case management. Effective verbal and written communication skills. Competent in care of infant, toddler, child, adolescent, adult and geriatric. Ability to interact well with external and internal customers. High level of clinical skills and ability to communicate clearly and succinctly in a customer focused professional manner.

Notes: Work days may vary with rotating weekends. Heavy paperwork and travel in the Riverside/San Bernardino counties.
